What is a radical prostatectomy?
It is the complete removal of the prostate together with the seminal vesicles, and it is the reference surgical treatment for localized prostate cancer. Depending on the risk of the tumor, it may be combined with the removal of the pelvic lymph nodes.
It should not be confused with surgeries for an enlarged prostate, such as prostate laser surgery or transurethral resection, which treat benign hyperplasia and do not remove the gland. They are different operations for different diseases.
When it is recommended and what the alternatives are
It is considered for cancer confirmed by biopsy and confined to the gland, in patients with a life expectancy long enough to benefit from it. The decision takes into account the PSA, the grade group, the stage, the MRI and your general health.
Active surveillance
Monitoring the tumor with PSA, rectal exams and MRI, without treating it as long as it does not progress. It is the first option for low risk.
Radical prostatectomy
Removes the gland. It is an option with curative intent for intermediate and high risk, in the latter as part of a plan that combines several treatments.
Radiotherapy or brachytherapy
The alternative without surgery, with or without hormone therapy depending on the risk.

Radical prostatectomy techniques
Long-term results of the open and the robotic techniques are similar; the minimally invasive approach usually offers a faster initial recovery.
Laparoscopic
Through small incisions, with long instruments and camera-guided vision. Less bleeding and a faster recovery than open surgery.

Continence, erections and fertility: what to expect
This is the main concern before this surgery. The priority is always to remove the cancer completely; the effects depend on the extent of the tumor and on each patient.
Continence
It is common to need protection during the first weeks. Control improves gradually over weeks or months, and pelvic floor exercises help. If leakage persists, there are diagnostic studies and treatments.
Erectile function
It depends on whether the nerves responsible for erections were preserved. Recovery can take from a few months to two years or more, and rehabilitation starts early. In severe cases, a penile prosthesis is an option.
Fertility
After surgery, orgasm is dry and natural conception is no longer possible. If you wish to have children, it is advisable to freeze sperm before the operation.
