What is erectile dysfunction and why does it happen?
It is the persistent difficulty in achieving or maintaining an erection sufficient for satisfactory sexual intercourse. An occasional bad night is not erectile dysfunction; it is when it recurs and affects your sex life or your peace of mind.

Erectile dysfunction treatments, step by step
Treatment starts with the least invasive option and moves forward if the response is not sufficient. None is indicated without a diagnosis, and in all of them habits and risk factors are corrected at the same time.
First line
Oral medication
Sildenafil, tadalafil and similar drugs make it easier to get an erection with sexual stimulation. They are taken with a prescription and never together with nitrates.

Low-intensity shockwave therapy
Office sessions for mild vascular causes, or when pills are not enough or cannot be used.
Weak recommendation in the European guideline; the American guideline (AUA 2018) considers it experimental.
If pills do not work
Intracavernosal injections
In the office, the patient learns to inject alprostadil into the penis, a medication that produces an erection directly.

Penile prosthesis
An implant inside the penis that allows an erection whenever the patient decides. It is the step with the highest satisfaction, and it is irreversible.

A penile prosthesis is an implant placed inside the corpora cavernosa that allows an erection whenever the patient decides. The European guideline recommends it when other treatments fail or if the patient prefers it, after complete information.
It is a permanent decision: although the implant can be removed, the penis no longer responds reliably to other treatments. That is why the prior assessment, ideally as a couple, is decisive.

Two flexible cylinders that keep the penis firm at all times; they are bent upward for intercourse and downward the rest of the day. It is simpler to use and to place.

No trials have directly compared satisfaction with the two types; the inflatable prosthesis is usually preferred for its more natural erection. The urologist recommends the type based on your manual dexterity, your anatomy and your expectations.

- Men with erectile dysfunction who did not respond to pills or injections, or who do not tolerate them
- Dysfunction of organic origin, for example due to long-standing diabetes or after prostate surgery
- Those who, well informed, prefer a definitive solution from the start
