What is andrology?
Andrology is the branch of medicine that studies men's sexual and reproductive function. In practice it is carried out by urologists with additional training in male fertility, hormones and sexual health.
Consulting about these topics is as routine as a gynecological visit for women. The assessment begins with a conversation and a physical exam, and tests are requested according to the reason for the visit: not every man needs the same ones.

Kidneys, bladder, prostate and urinary tract in men and women, as well as the male reproductive system. Stones, prostate, infections, incontinence and urological cancer.
Andrologist
A urologist dedicated to men's sexual and reproductive health: infertility, low testosterone, premature ejaculation and erectile dysfunction. Works hand in hand with the couple's fertility team.

Step 1
Consultation and medical history
Time trying to conceive, sexual life, medications, surgeries and history. It is done in private and confidentially.
Step 2
Physical exam
Testicles, epididymis and spermatic cord; this is where a palpable varicocele is detected.
Step 3
Semen analysis
The basic test for the male factor. If it is abnormal, it is repeated.
Step 4
Hormone panel
Total testosterone, FSH, LH and prolactin, taken in the morning, when the case calls for it.
Step 5
Targeted studies
Testicular ultrasound, semen culture, DNA fragmentation or genetic tests, only if the case requires it.

Preparing for the semen analysis

They are the 5th percentile of men whose partners achieved a pregnancy in 12 months or less. A value below them does not mean sterility, and a normal one does not guarantee fertility: the result is interpreted at the consultation, together with the couple.
Infertility is also a man's issue
The couple's work-up starts with both partners at the same time: the man's is simpler and faster.

Of men who consult for infertility have it, compared with about 15% of the general population (EAU).
When to get tested
- After 12 months of regular unprotected intercourse without achieving a pregnancy
- At 6 months if the partner is 35 or older
- The man and the woman are evaluated at the same time; the semen analysis can be done before that period
Causes that are looked for
- Varicocele: dilated veins in the scrotum that can affect the semen; a testicular Doppler ultrasound can show it
- Hormonal disorders, such as low testosterone or high prolactin
- Blockage of the ducts that carry sperm
- Treatments such as chemotherapy, and the use of testosterone or anabolic steroids

Low testosterone: how it is confirmed
The diagnosis requires symptoms and a low value confirmed in the laboratory. It is not treated for tiredness alone.

Below this value, measured in the morning on two different days and with symptoms, it is considered testosterone deficiency.
Symptoms that point to it
- Less sexual desire and less firm erections
- Lack of energy and less strength or endurance
- Low mood or irritability
- Loss of muscle mass or of underarm and pubic hair
Before treating
- It is confirmed with a second morning measurement
- LH is measured and, if it is low, prolactin, to find the cause
- If you are hoping to have children, testosterone is not used as a treatment because it suppresses sperm production; there are alternatives, such as clomiphene, under medical supervision
When it is considered premature ejaculation
The International Society for Sexual Medicine (ISSM) defines it with three criteria that must be present at the same time.
Short time
About 1 minute or less after penetration if it has occurred since the start of sexual life, or a clear reduction, to 3 minutes or less, if it appeared later.
Lack of control
Inability to delay ejaculation in all or almost all encounters.
Distress
Negative personal consequences: distress, frustration or avoiding intimacy.
In the surveys cited by the European guideline, the lifelong form affects about 2% to 3% of men and the acquired form about 4%; rapid ejaculation only on some occasions, which does not meet the criteria, is more common.
Treatment
The European guideline combines behavioral and couple techniques with medication. The first line in the lifelong form is oral dapoxetine or an anesthetic spray of lidocaine and prilocaine; the specialist chooses the option with you.
