What is a cystoscopy?
A cystoscopy is the test that lets the doctor see inside the urethra and the bladder. The urologist inserts a cystoscope, a thin tube with a camera and a light, through the urethra. It is the only method that allows a direct look inside the bladder, something that ultrasound and CT scans cannot do.
There are two types of cystoscope. The flexible one is the most common for the diagnostic test and is used under local anesthesia in the office. The rigid one allows biopsies and treatments, and requires regional or general anesthesia.
A common confusion is worth clearing up: a cystoscopy reaches only the bladder, while a ureteroscopy goes up the ureter to the kidney and is used mainly to treat stones.
When is a cystoscopy recommended?
Most patients come because of a finding in another test. These are the most common reasons.
Blood in the urine
Visible or detected in a test. It is the most important indication and the one that should not be postponed.
Recurring urinary infections
When they come back again and again with no clear cause.
Persistent urinary symptoms
Urgency, pain when urinating or difficulty emptying the bladder, with no explanation.
Suspected bladder tumors or their follow-up
It is the reference test to see them directly.
Stones, strictures or foreign bodies
Both to study them and to treat them.
Follow-up after bladder treatment
To check that there is no recurrence.
Does a cystoscopy hurt?
During the test
With a flexible cystoscope and local anesthetic gel, most patients describe discomfort, burning or an urge to urinate rather than pain. With a rigid cystoscope and anesthesia, nothing is felt during the procedure.
In the following days
It is normal to feel burning when urinating for one or two days and to see slightly pink urine. Drinking plenty of water helps. See a doctor if you develop a fever, severe pain or are unable to urinate.
Practical information
A urinalysis is usually requested beforehand to rule out an active infection: if there is one, the test is postponed. The imaging test that usually accompanies it is the urinary tract ultrasound.
