What neuro-ophthalmology is
It is the subspecialty that studies vision problems caused by the optic nerve, the visual pathways of the brain and the nerves that move the eyes. A neuro-ophthalmologist is an ophthalmologist with one or two years of additional training in this field. In Spanish, some clinics spell it neuroftalmología, with a single o: it is the same thing.
Being referred does not mean you have something serious in your brain. Often the goal is to rule things out. The neuro-ophthalmologist brings together the eye exam, the visual field and the imaging, and decides whether management is ophthalmological, neurological or joint. In general it is a specialty that does not operate.

Conditions a neuro-ophthalmologist treats
These are conditions in which vision fails even though the eye looks normal from the outside.
Optic neuritis
Inflammation of the optic nerve: reduced vision in one eye, pain when moving it and washed out colors. It can be associated with multiple sclerosis or autoimmune diseases.
Double vision (diplopia)
The muscles or nerves that move the eyes stop working in coordination. It can come from the cranial nerves, the thyroid, myasthenia or a brain lesion.
Papilledema and intracranial hypertension
Swelling of both optic nerves due to increased pressure inside the skull. Idiopathic intracranial hypertension mainly affects overweight women aged 20 to 40, with headache and blurred vision.
Ischemic optic neuropathy
Lack of blood supply to the optic nerve: sudden, painless vision loss, more common after age 50.
Unequal pupils and drooping eyelid
When they have a neurological cause, as in Horner syndrome or myasthenia. If the eyelid droops gradually because of the skin or the muscle, it is handled by oculoplastic surgery.
Visual field loss
Areas where you cannot see, to one side or in the center. The specialist looks for whether the cause lies in the nerve or in the visual pathway. Glaucoma also damages the optic nerve and is told apart at the assessment.

Neuro-ophthalmology tests
The specialist chooses which ones you need based on the exam findings. Not every test is done in every case.
How they are treated, depending on the cause
Treatment depends on what the specialist finds and is often decided together with neurology.
Optic neuritis
Vision often returns to normal within 2 to 3 weeks, even without treatment. Corticosteroids can speed up recovery, although the final result is similar without them. It is decided with neurology.
Intracranial hypertension
The cause is treated: weight loss, medications such as acetazolamide or topiramate and, in selected cases, a shunt.
Double vision
Prisms in the glasses, management of the underlying condition such as thyroid disease or myasthenia and, if it persists, eye muscle surgery or botulinum toxin. Eye muscle surgery is coordinated with strabismus treatment.
Optic nerve monitoring
Repeated visual fields and OCT scans to monitor the nerve over time and adjust treatment.
